Is this too long? An option would be to make the categories (adult-minor sex, etc.) into collapsed, expandable boxes.

- It may look better collapsed. That isn't PNVD's logo btw, just an impression of it cut and pasted from their website. Maybe Kinsey would be more fitting at the top of this template?
